About Community VNA Hospice
Our Hospice and Palliative Care Programs are specialized programs that provide advanced care for individuals and their families who are facing any type of serious, life-limiting illness. We offer dignified, compassionate end of life care that helps patients and their loved ones face life-limiting illnesses. Community VNA’s Hospice and Palliative Care nursing staff are available 24 hours a day, 365 days a year, to provide skilled medical home care and assistive support, to treat physical symptoms as well as the emotional and spiritual needs of our patients and their loved ones.
