ACCIDENT WAIVER AND RELEASE OF LIABILITY I acknowledge that the Sevier Valley Rooster Ride event for which I will participate, whether as a participant, contestant, entrant, volunteer or spectator or a combination of the foregoing, is a test of my physical and mental abilities and carries with it the potential for significant injury, property damage or loss, or death. I acknowledge there are many risks associated with participating in this event, including but not limited to weather conditions, road conditions, course difficulty, condition of my equipment, vehicle traffic, lack of hydration, actions of other people, my own physical condition (participant), and other unforeseen hazards. I hereby assume all of the risks of participating in any way in this event. I hereby certify that I am physically fit and have sufficiently trained and prepared for participation in this event. If I participate in this event as a bicyclist, I will wear an ANSI-approved helmet at all times while riding in the event and will abide by all state and local laws at all times. I acknowledge that cycling is a dangerous sport/activity in which I participate at my own risk.
In consideration of the agreement of Sevier Valley Rooster Ride to allow me to participate in the event, either as a participant, contestant, entrant, volunteer or spectator, I hereby, for myself, my heirs, assigns and personal representatives, waive all claims against, and release and forever discharge, Sevier County, Sevier County Rooster Ride, its owners, members, managers, employees, officers, directors, agents, sponsors, promoters, volunteers and affiliates whosoever from any and all liabilities, damages, claims, losses, costs or expenses arising from or attributable in any way to any act or omission to act relating to my participation in the event in any manner, including my traveling to and from the event. I have no physical condition that would interfere with my ability to participate in or attend the event or would endanger my life thereby. I hereby consent to receive any medical treatment which may be deemed advisable in the event of injury, accident or illness during the event.
I grant permission to use my photograph, motion picture, recordings or any form of record for this event for any legitimate purpose by the event holders, producers, sponsors, organizers and or assigns. By signing this waiver, you agree to allow Sevier County Tourism to contact you via email with event and county tourism information.
I hereby certify that I have read this document and, I understand and agree to its content. General Waiver on behalf of Minor MINOR'S PARENT / LEGAL GUARDIAN, the minor's parent or legal guardian, understand the nature of bicycling and the minor's experience and abilities and believe the minor to be physically capable of participating in this activity. I assume the responsibility for any injury that the minor may suffer, and I hereby make the foregoing waivers and releases on behalf of the minor and myself.
